Select the first option </strong>(I / We wish to make a nomination)<br><strong>2. Name, UCC, Demat ID</strong>(which can be found under Investment trading account section of profile page in your PhonePe Invest app)<br><strong>3. Nominee details:</strong><br>• Name<br>• Share of each nominee<br>• Relationship<br>• Address (incl City/Place, State/Country , PIN Code)<br>• Mobile no of nominee<br>• Email id of nominee<br>• Nominee identification details (one among PAN/Aadhaar/DL)</p>\n\n\n\n<p>If <strong>nominee is a minor </strong>(less than 18 years) update their DOB, guardian details (Name, Relationship) as requested&nbsp;</p>\n\n\n\n<p>Select one option between <strong>Name of Nominee / Nomination</strong>: Yes/No to be printed in the holding/account statement (Page 3 &#8211; first option)</p>\n\n\n\n<p>Sign the form</p>\n\n</div></div>\n\n\n<p>Once signed, you can courier the form to the below address:</p>\n\n\n\n<p>PhonePe Wealth Broking Pvt Ltd, Office &#8211; 2, Floor 3, Wing A, Block A, Salarpuria Softzone, Bellandur Village, Varthur Hobli, Outer Ring Road, Bangalore South, Bangalore, Karnataka &#8211; 560103, India</p>\n\n\n\n<p><strong>Note</strong>:&nbsp;</p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Nomination is <strong>optional</strong>.
