The certificate must state the percentage of disability.</li><li>Notarised and self-attested copy of the certificate of guardianship.</li><li>Passport-size photographs of the client and the guardian to affix and sign across on the applications.</li><li>Self-attested PAN copies of the client and guardian with the signature under applicant signature.</li><li>Self-attested address proofs (Either driving licence, voter ID, passport, or Aadhaar card) of the client and guardian.</li><li>A bank account proof in the client&#8217;s name.
